Once you have completed the registration form below, you will receive a confirmation from us shortly and will be able to schedule an intake appointment with us.
If you want to register your whole family with us, we kindly ask you to fill out a separate form for each family member. Please indicate the corresponding family for each member in the "Message" field.
Due to increasing demand, we are currently only accepting patients from the following postal code areas:
2551, 2552, 2553, 2554, 2555, 2564, 2565, 2566 and Westland.
This is to ensure we can maintain the quality of our care.
If you fail to cancel your appointment at least 48 hours in advance, we reserve the right to charge you for the cost of appointment. If frequent cancellations occur, it reduces the time available for other patients.
We have outsourced the processing of our invoices to Infomedics. This factoring company will collect the insured portion of your invoice from your insurance provider, provided you have dental insurance.
You will receive remaining balance invoice from Infomedics for the portion you need to pay yourself.
If you do not have dental insurance or if the amount exceeds the maximum coverage of your insurance, we kindly request that you make the payment directly after the treatment.
